Registered agent annual fee, state report fee, and the letter that asks for both
They are two different bills. Your registered agent charges a yearly service fee (Mainstay Filing: $149, the same at renewal). The state charges a yearly or two-yearly report fee, $7 to $500 where it charges one, and 6 states charge none. A letter that sounds official and wants more than the state’s own fee is not the state’s bill.

Questions
Is this annual report letter a scam?
If the sender is not your state’s filing office, it is a private company offering to file for a fee, not a government bill. Some are lawful offers that say so in small print; the harm is that they look official. Compare the amount with the state’s own fee in the table on this page before you pay anything.
Is my registered agent’s annual fee the same as the annual report fee?
No. The agent’s fee is the service price, $149 a year at Mainstay Filing. The report fee is paid to the state: $7 to $500 where one exists, and 6 states have no LLC report at all.
Who files my annual report?
You, your registered agent if it offers that, or a filing service. Mainstay Filing prepares and files it inside the $199 a year formation package; the state fee is passed through at cost.
